His cousin, Mary Milligan, wrote that he "was participating in a Triathlon Race sponsored by the University Hospital in Boulder, Colorado. He collided with another bike rider and fell under a passing truck. Steven lived in Colorado and loved it and he is still there. His ashes are over the highest peak at Vail, Colorado. 'The Steve McAdams Memorial Triathlon Race' was held at Lakewood, Colorado 22 Aug. 1988."
